Win / Loss Requests

In order for Boomtown New Orleans to release information regarding your tracked play you must complete and submit a signed W-2G/Win-Loss Request Form.

Once the request form is completed you can submit the form for processing in one of three ways:

1. By mail to the address listed on form
2. By facsimile to the number listed on form
3. In-person

to download the Win/Loss Request Form.

Please allow up to 2 weeks for request processing.
